The UPS plane involved in Tuesday's fatal crash in Louisville Kentucky had recently completed a long period of maintenance in San Antonio, where it spent most of September and October.
FAA records indicate that during this time 03 SEP to 18 OCT 2025, workers discovered and reported significant issues, including rust on structural parts in two separate areas in the bilge area of the plane. They also found a cracked stringer near the center wing upper fuel tank, before the MD-11 was returned to service.
Federal investigators will now review those maintenance records to determine whether mechanical issues played a role.
Initial findings showed that the left wing caught fire while the plane was rolling toward takeoff, and its engine fell off.
Investigators say they have recovered the flight recorder from the wreckage.
